It was not the air traffic controllers fault the Dutch pilot took off when he was not meant to, The airport also was not equipped to deal with that much air traffic as it was a small regional airport. The final report laid the blame squarely at the Dutch pilot but the Dutch investigators refused to acknowledge this so they released their own report.
